The following is a list of issues addressed in R4+ v8.1.19. All fixes are cumulative (each new release contains all new fixes as well as all fixes from the previous release).
Where a Patient undergoes Supervision of Retention stemming from completed orthodontic treatment, this is considered part of the same originating course of treatment in relation to the Patient’s Legal Maximum Charge for Treatment.
In some cases, if the patient is close to the Maximum Charge Limit in an originating course of treatment, this could result in their subsequent Retentions continuing to charge the patient when they have crossed the maximum 80% legal limit. We have introduced this tool to make it easier to manage the applied limit in these situations.
The Orthodontic Linked Patient Charge Manager can be accessed via the Patient Record or the Clinical Chart, and allows the user to identify a base course of treatment, Retainer treatment, and subsequent Retentions, and be able to link them via a drag and drop interface. The Overall Patient Charge and the Remaining Limit is presented to aid the user.
Once used, if a patient has an open orthodontic course of treatment which has a linked Patient Charge applied, this will be identifiable in the Patient Banner by a new chain link icon and tool-tip. This will indicate to any user accessing the patient why they may see a different charge behaviour in any active orthodontic course of treatment to what they would normally expect to see, due to observing the legal limit across the linked treatment plans.

A defect where the 1441 (Fissure Sealant) Code had been incorrectly categorised as a Sealant Restoration has now been re-categorised as a Fissure Sealant (Preventative). This allows the treatment to maintain a Band 1 NHS Charge value without being affected by the Diversification of NHS Charge Band 2 in England as this treatment does not include a restorative action, unlike Fissure Restorations. This has been fixed in R4+ v8.1.19.
A defect caused R4+ to set any Sealant Restoration Treatments (covering codes 1442, 1443, 1444 & 1461) as ‘Other’ in FP17 Claim CDS Information, which also gave an inaccurate representation of the completed treatment. This could also result in rejected claims from COMPASS due to having no recognised specific treatments having been performed for processing.
R4+ will now correctly set both the Permanent Filling and Fissure Sealant CDS Values when these treatments are completed to signify the provision of both a restoration to the tooth due to caries and a subsequent sealant being applied at the same time, after consultation with the BSA on the Correct CDS Transmission for Sealants which has now been fixed in R4+ v8.1.19.
