Evaluating and Marking the List

Evaluate each item on the list to determine whether it is appropriate for your practice, or if it should be changed. Delete procedures that are not performed in your practice.

Draw a line all the way across the page through the procedures you want to delete. Use a highlighter to mark any code descriptions you want to change. Each description can be up to 25 characters, spaces and punctuation included.
