Using the Questionnaire Editor

To create a new patient questionnaire using the Questionnaire Editor:

  1. Select List > Patient Questionnaire. The Patient Questionnaire List window is displayed in FIND mode.

  2. Click Add. The Patient Questionnaire window is displayed in ADD mode. The questions for all categories in other questionnaires are displayed.

  3. Type the questionnaire title in the Test Name field.

  4. Select the category of questions to display in the Questions section from the Category drop-down list. Select All to include questions from all categories in the list.

  6. To add an existing question to the new questionnaire:To add an existing question to the new questionnaire:Select the question from the Questions section and click Selected in the Add >>>> section.

  7. To add consecutive questions to the new questionnaire:To add consecutive questions to the new questionnaire:Select the first question, press Shift, and select the last question. Click Selected in the Add >>>> section.

  8. To select non-consecutive questions to the new questionnaire:To select non-consecutive questions to the new questionnaire:Select the first question, press Ctrl, and select each question. Click Selected in the Add >>>> section.

  9. To add all existing questions to the new questionnaire:To add all existing questions to the new questionnaire:Click >>All in the Add >>>> section.

  1. To remove a question from the Questions Selected section, select the question and click Selected in the Remove < section.

  2. To remove all questions from the Questions Selected section, click <<All in the Remove < section.

  1. To create a new question, click Add Question. The Question window is displayed.

  2. Type the question in the Question field.

  3. Select an option:

  4. To add the new question to a new category:To add the new question to a new category:Type the category name in the Category drop-down list.

  5. To add the new question to an existing category:To add the new question to an existing category:Select the category from the Category drop-down list.

  2. If the answer to the question is yes or no:If the answer to the question is yes or no:Select Yes/No.

  3. If the answer to the question is free-form:If the answer to the question is free-form:Select Free Form.

  4. If the answer to the question is numeric:If the answer to the question is numeric:Select Numeric.

  5. If the answer to the question is to be selected from a list of responses:If the answer to the question is to be selected from a list of responses:Select List. The Enter a Response section of the window is displayed. Type a response in the field located at the top of the section, and click Add Response To List.

    Repeat until all response choices are entered. To edit or delete a response, select it and click the appropriate button. To change the display order of the responses, select a response and click the appropriate button.

  6. If the patient can select more than one answer from a list of responses:If the patient can select more than one answer from a list of responses:Select Multiple List. Type a response in the field located at the top of the section, and click Add Response To List.

    Repeat until all response choices are entered. To edit or delete a response, select it and click the appropriate button. To change the display order of the responses, select a response and click the appropriate button.

  1. After you have finished editing the question, click OK. The Patient Questionnaire window is displayed.

  2. To add the new question to the questionnaire, select it from the Questions section of the window and click Selected in the Add >>>> section.

To set a question dependency:To set a question dependency:Select the parent question from the Questions Selected section, press Ctrl, select the child question, and click Make in the Dependencies section. The Question Dependency window is displayed. The child question is displayed in the first field, and the parent question is displayed in the second field.

    To set the dependency, select an option and click OK. The dependency information is displayed in the DEP column of the Questions Selected section.

  1. You can have multiple parent child relationships, but you must set each one up separately.

  1. Click OK.
