Handling Patient Questionnaires

Entering Patient Questionnaire Responses

After a questionnaire has been completed by a patient, you can enter the results into the software for tabulation.

To enter patient questionnaire responses:

1. Select an option:

2. In the Patient List window:Select the patient and select Other Patient Management > Patient Questionnaire from the Options section of the Express bar or Options menu.

3. In the Patient window:Select Other Patient Management > Patient Questionnaire from the Options section of the Express bar or Options menu.

The Patient Questionnaire Results List window is displayed.

2. Select Enter New Test Results from the Options section of the Express bar or Options menu. The Patient Questionnaire Results window is displayed.

3. Select the questionnaire from the Questionnaire drop-down list.

4. Select the administration date from the Date Administered drop-down list.

5. Click OK. The Questionnaire Responses for window is displayed.

6. Select the answer and click Next.

7. Click OK.
