Using the Patient Questionnaire Creation Wizard

To modify existing questionnaires, see Editing Patient Questionnaires.

To create a new patient questionnaire:

  1. Select Wizards > Questionnaire Creation Wizard.

  2. Read the introductory statement and click Next.

  3. Type the question title in the field and click Next.

  4. To add a question that exists on another questionnaire to the new questionnaire, select I would like to add an existing question to the questionnaire and click Next.

Select a question category from the list and click Next.

Select a question from the list and click Next. Go to step 10.

  1. To add a new question to the questionnaire, select I would like to create a new question and add it to the questionnaire and click Next.

  2. Select an option:

  3. To create a new category:To create a new category:Type the category name in the top field and click Next.

  4. To use a previously created category:To use a previously created category:Double-click the category name in the bottom section of the window and click Next.

  1. Type the question in the field as it is to print on the questionnaire and click Next.

  3. If the answer to the question is yes or no:If the answer to the question is yes or no:Select Yes/No - This is a "Yes/No" question and click Next. Go to step 10.

  4. If the answer to the question is free form:If the answer to the question is free form:Select Free Form - This is a "fill-in-the-blank" question and click Next. Go to step 9.

  5. If the answer to the question is numeric:If the answer to the question is numeric:Select Numeric - The answer can be a positive number and click Next. Go to step 10.

  6. If the answer to the question is to be selected from a list of responses:If the answer to the question is to be selected from a list of responses:Select List - The user can pick one answer from a list of responses and click Next.

  7. If the patient can select more than one answer from a list of responses:If the patient can select more than one answer from a list of responses:Select Multiple List - The user can pick zero or more answers from a list of responses and click Next.

  1. Select an option:

  2. To enter an answer to the question:To enter an answer to the question:Type it in the field in the top section of the window and click Add or press Enter. The answer is displayed in the list in the bottom section of the window.

  3. To edit an answer to the question:To edit an answer to the question:Select it from the bottom section of the window and click Edit. Make the changes and click Add.

  4. To delete an answer from the list:To delete an answer from the list:Select it from the bottom section of the window and click Remove.

  1. After you have entered all of the possible answers to the question, click Next.

  2. Review the questionnaire and click Next.

  3. Repeat steps 4 through 11 until you have added all of the questions to the questionnaire.

  4. After you have added all of the questions to the questionnaire, in the third window of the Patient Questionnaire Creation Wizard select I am done adding questions to the questionnaire and click Next.

  5. Select an option:

  6. To complete the patient questionnaire:To complete the patient questionnaire:Click Finish. Go to step 19.

  7. To create question dependencies:To create question dependencies:Click Next. The text and type of each question are displayed in the list.

  1. Select the parent question from the list, press Ctrl, and select the child question from the list. The child question is dependent upon the answer of the parent question.

Example:Example:Question 10 is asked only if the answer to question 9 is Yes.

  1. You can have multiple parent/child relationships, but you must set each one up separately.

  1. Click Next.

  2. To set the dependency, select an option and click Next.

  3. Repeat steps 4 through 17 until you have added all of the dependencies to the questionnaire.

  4. Click Finish.

  5. Review the questions, and click Save.
