Editing Patient Questionnaires

You can edit patient questionnaires after they have been created and after responses have been entered into the software.

To edit a patient questionnaire:

  1. Select List > Patient Questionnaire. The Patient Questionnaire List window is displayed in FIND mode.

  2. Double-click the questionnaire.

  3. Select an option:

  4. To add an existing question to the questionnaire:To add an existing question to the questionnaire:Select the question from the Questions section and click Selected in the Add >>>> section.

  5. To add a new question:To add a new question:Click Add Question. See Using the Questionnaire Editor.

  6. To edit a question:To edit a question:Select the question from the Questions Selected section and click Edit Question. If the question is the parent of a dependent (child) question, the dependency will be broken when you edit the parent question. The Question window is displayed.

    Make the changes and click OK. If the meaning of the question is changed, tabulated responses for the original question are retained and can be viewed in the Questionnaire Results window. INACTIVE is displayed in the Status column of the original question.
    If the meaning of the question is not changed, tabulated responses for both questions are stored together.

  1. Click OK.

  2. Click Close.
