This report indicates a rejection of a claim by United Concordia. The patient’s last name and claim total are listed, along with the data that was found in error at United Concordia.
To receive this report, you must have loaded, validated, and transmitted your claims to the eServices clearinghouse.
This report is returned by the insurance company within one to three business days. It lists the claims sent during the previous session.
This report indicates that a problem was encountered processing a recently received file. This could be due to an interrupted transmission or corrupt data. Contact eServices whenever this error occurs.
