Attaching Forms

Click Forms to display a list of forms.

When you select a form from the list, it automatically attaches to the tooth or teeth selected in the mouth area. Use the Define new or Edit form buttons at the bottom of the list to create or customize a form.

The Form Definition window includes the following options:

Group
Form name
Description for database
Category
Form type
Add question
Remove question
Edit question
Test
OK
Cancel
