Configuring OrthoTrac > Setting Up Statement Options
To access the header tab:
Select Functions > Maintenance/Set-up > Statement Options from the menu bar.
In the Statement Options window, click the Header tab.
On the Header tab, select from the following options:
Include Address at TopInclude Address at Top
Select this option to print the office location return address in the upper-left of the statement, for use with window envelopes.
Include Insurance InformationInclude Insurance Information
Select this option to print the name of the insurance carrier, responsible party, and employer. If Include Insurance Information is not selected, this information is not printed on the statement, but any estimated insurance balances print on the statement if Include Estimated Insurance Amounts is selected from the Body tab.
