Running the Provider by ID Report

The Provider by ID report generates a list of providers in the database. The list is sorted by identification number.

For each provider, the report includes:

• Identification number

• Name

• Address

• Social Security number

• Tax identification number

• License number

Sample Report

To run the Provider by ID report:

1. Select Reports > Provider/Referring Dr > Provider > ID. The Output Options window is displayed.

2. Select an output option and click OK. The Provider By ID Report Setup window is displayed.

3. Select the start and end provider identification numbers.

4. To print the provider’s home address in the report, select Use Provider’s Home Address.

5. To use color shading in the printed report, select Include Color Shading.

6. Click OK.
