Adding or Removing eForms

You can add or remove eForms for a specific appointment. Forms added or deleted here are exceptions to the forms a patient should receive, as specified in the default eForms template settings.

Exceptions to the normal qualified eForms are highlighted with either red or green. Red indicates eForms that have been deleted from the list of qualified eForms. Green indicates eForms that have been added to the list of qualified eForms. Light blue indicates a template is selected.

To add or remove eForms for a patient:

 

In the Scheduler, click on an appointment, and select eForms > Add/Remove eForms from the options panel at the left side of the window. The Appointment eForms Templates window is displayed.
