Handling Patient Questionnaires

Viewing and Printing Patient Questionnaires

To display or print a patient questionnaire:

1. Select an option:

2. In the Patient List window:In the Patient List window:Select the patient and select Other Patient Management > Patient Questionnaire from the Options section of the Express bar or Options menu. The Patient Questionnaire Results List window is displayed. Select the questionnaire and select View a Completed Test from the Options section of the Express bar or Options menu.

3. In the Patient window:In the Patient window:Select Other Patient Management > Patient Questionnaire from the Options section of the Express bar or Options menu. The Patient Questionnaire Results List window is displayed. Select the questionnaire and select View a Completed Test from the Options section of the Express bar or Options menu.

4. Select List > Patient Questionnaire:Select List > Patient Questionnaire:The Patient Questionnaire List window is displayed. Select the questionnaire and select Print from the Options section of the Express bar or Options menu.

The Output Options window is displayed.

2. Select the option and click OK.
