Tabulating Questionnaire Results by Questionnaire

To tabulate patient questionnaire results by questionnaire and run the Tabulate Results by Questionnaire report:

1. Select Reports > Patient > Patient Questionnaire > Tabulate Results By Questionnaire. The Output Options window is displayed.

2. Select the output option and click OK. The Select a Questionnaire window is displayed.

3. Select the questionnaire from the Questionnaire drop-down list.

4. Select an option:

5. To view the summary for all responses:To view the summary for all responses:Select Summary.

6. To view the summary and details for responses:To view the summary and details for responses:Select Summary and Details.

6. To restrict the report to responses entered since the questionnaire was last edited:To restrict the report to responses entered since the questionnaire was last edited:Select Show Responses Since the Questionnaire was Last Modified.

7. To restrict the report to a specific date:To restrict the report to a specific date:Select the dates from the From and To date fields.

8. To include all responses:To include all responses:Select Show All Responses.

6. Click OK.
