Tabulating Questionnaire Results by Questionnaire

To tabulate patient questionnaire results by questionnaire and run the Tabulate Results by Questionnaire report:

  1. Select Reports > Patient > Patient Questionnaire > Tabulate Results By Questionnaire. The Output Options window is displayed.

  2. Select the output option and click OK. The Select a Questionnaire window is displayed.

  3. Select the questionnaire from the Questionnaire drop-down list.

  4. Select an option:

  5. To view the summary for all responses:

  6. To view the summary and details for responses:

  2. To restrict the report to responses entered since the questionnaire was last edited:

  3. To restrict the report to a specific date:

  4. To include all responses:

  1. Click OK.
