Selecting a Billing Statement

You can use a variety of statement forms.

To select a billing statement form:

  1. Select System > Form Selection. The Form Selection window is displayed.

  2. Select the billing statement and walkout statement forms from the Billing Statements and Walkout Statements drop-down lists.

  3. If you process claims electronically, select the form from the Electronic Statements drop-down list. For more information, see Setting Up  Automated Statements.

  4. Click OK.
