Handling Transactions and Codes > Posting Patient Procedures
You might encounter a situation where you need to remove an existing graphic from the chart and re-chart a new procedure. (For example: an implant fails and needs to be retreated; an extraction was charted for the wrong tooth, etc.) In a case like this, you can use the Remove Chart Graphic option on the Patient Transactions window.
You must have the "Modify Remove Chart Graphic Checkbox" Transactions security permission to use the Remove Chart Graphic feature.
When you edit a procedure code transaction and check the Remove Chart Graphic option:
A grey asterisk (indicating the procedure has been removed) is displayed next to the tooth number with the removed graphic.
A *Removed graphic for [ada code description] entry is placed in the Description column of the Clinical History tab for the transaction with the "Remove Chart Graphic" checkbox selected.
A Rem Cht Graph -- Checked entry is made in the Transaction Audit Trail report.
A Rem Cht Graph--Unchecked entry is made in the Transaction Audit Trail report when charting graphics have been added back into the chart (when the Remove Chart Graphic checkbox is deselected).
To remove an existing charted procedure from the chart:
In the Patient List window, select a patient and click Transactions on the toolbar. The patient Transactions List window is displayed.
Select the transaction for the procedure you want to remove from the chart, and click Edit. The Patient Transactions window is displayed.
Select Remove Chart Graphic, and type a reason for the change in the Note field.
Click OK.
Click Close.
Open the patient's chart and chart the new procedure. (Note the asterisk next to the tooth number and the "Removed Graphic" entry on the Clinical History tab.)
