Using the eForms Module

The eForms module enables you to make patient forms available electronically. Electronic forms can be e-mailed to patients to be filled out and submitted prior to an appointment, or queued up to be ready for completion on a workstation in your office when a patient arrives. You can also capture patient signatures electronically, eliminating much of the traditional office paperwork.

You can use eForms for patient information and medical history, insurance documents, consent forms, financial policy agreements, HIPAA acknowledgements, and other customized forms.

You can access eForms from the Activities menu, and from the context menu for an appointment.

From the Activities menu, select eForms, and then the action you want to perform:

From the Appointment Book, right-click an appointment to view the eForms options:

When you select an eForms action, the corresponding eForms window is displayed.
